Company Description Job Description Medical Social Worker - Hospice Our patients need assistance with community resources, grief counseling, caregiver assistance, and advanced directive and POLST preparation including funeral arrangements for end of life.

Assess emotional factors related to the illness.
Assess need for counseling for the patient and family Consult and collaborate with the interdisciplinary team to develop and revise a plan of care for the patient and family.
Act as resource to access community resources for the patient and family.
Support the patient s unique spiritual beliefs.
Accept other duties as assigned.
Qualifications:
Master of Social Work degree from a school of social work accredited or approved by the Council on Social Work Education.
Must have one (1) year of experience and have a Masters in Social Work.
